Board Certified in Internal Medicine, Dr. Jamal Lone, serves as the Medical Director for Autumn Leaves. In his role he oversees onsite medical care for our skilled nursing residents. He and his team are able to provide convenient access to medical care while managing their conditions on a day-to-day basis.
Residents are cared for in the convenience of their apartments and are treated by the same medical team allowing for continuity of care, which can lead to better outcomes.
One of the key benefits of care being managed by a medical director and his healthcare team is the fact that that residents’ condition is closely monitored, and changes can be quickly identified to help prevent hospitalizations and ER visits. The team is dedicated to improving medication and disease management for our residents.
